Duties
- Deliver comprehensive patient care tailored to individual needs
- Develop and implement personalized care plans
- Conduct thorough assessments utilizing knowledge of physiology, anatomy, and medical terminology to inform treatment strategies (when applicable)
- Assist with discharge planning by coordinating with healthcare teams to ensure smooth transitions from hospital or skilled nursing facilities to home environments (when applicable)
- Perform skilled nursing duties including medication administration (when applicable), wound care (when applicable), and patient education
- Document all patient interactions accurately while adhering to regulatory standards and best practices
Qualifications
- Valid licensure or certification as an RN, PT, PTA, OT, COTA, ST, or HHA
- Proven experience in home health settings or skilled nursing facilities
- Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques (when applicable)
- Ability to develop effective care plans and participate in discharge planning processes (when applicable)
- Excellent communication skills and a compassionate approach to patient care
- Bilingual candidates are highly encouraged

- Zone 6: Tamarac, North Lauderdale, Coral Springs, Margate, & Parkland
- Zone 7: Pompano Beach, Coconut Creek, Deerfield Beach, & Hillsboro Beach
