Responsible for the provision of social work services to clients of the Albert Pujols Wellness Center for Adults with Down Syndrome Assess, plan and facilitate for options and services to meet an individuals unique and complex need. This is accomplished by addressing both the individual patient's biopsychosocial needs as well as the resources available. These services are offered/provided across the continuum. Responsible for planning and implementing the INSPIRE programs which are offered to both clients and parents. Continuing commitment to maximizing the quality of life for each patient and enhancing community health.

1. Interviews all clients and their parent/caregiver to assess wellness needs. 4%
2. Completes psychosocial assessment consistent with department guidelines. The assessment serves as a tool to determine baseline skills, behaviors, strengths and concerns. This assessment serves as a tool in defining wellness needs and appropriate wellness programs within and outside of the center. 20%
3. Serves as a member of an interdisciplinary care planning team; reports on client progress makes recommendations concerning changes in the care plan. 4%
4. Observes client's behavior and participation in wellness activities and makes recommendations to interdisciplinary care planning team. 4%
5. Provides progress reports to parent/caregiver on a regular basis for those clients who are active participants of wellness center sponsored classes. 4%
6. Develops and implements performance improvement monitors to assess. 4%
7. Provides brief, targeted therapeutic interventions for identified problems. Focus on the adaptive capacities of patients/families/caregivers with the goal of preventing maladaptive behavior and enhancing recovery. Appropriately assesses and intervenes in situations of abuse, neglect, domestic violence and sexual assault. 4%
8. Is knowledgeable of community resources and utilizes resources wisely. Develops and maintains professional relationships with community resources. Serves on committees as appropriate. 10%
9. Serves as a resource for Wellness Center Program development. Identifies strategies, topics, and speakers consistent with needs of population served. 20%
10. Maintains knowledge base through continuing education (per department standards) and applies knowledge to current practice of social work and health care. 4%
11. Able to define patient continuum of care needs. Requires knowledge of human development and behavior, of social, economic and cultural institutions and their interaction as part of the patient/family system. 4%
12. Responsible for coordination and implementation of discharge/continuum of care needs from preadmission or admission to assure identified needs are addressed. Identifies appropriate education needs of patients utilizing appropriate in house and community resources. Updates and maintains plan of care to reflect changing care needs of patient. Facilitates post discharge arrangements. Oversees that they are carried out in a safe and appropriate manner. Communicates plans, changes to healthcare team members. 4%

16. Participates in department PI activities to ensure and enhance the quality of patient care and social work services. 2%
17. Provides Advance Directive Counseling and can serve as witness. 2%

Employee is required to sit frequently and walk occasionally. Employee must lift/carry a light (1-20 lbs.) load occasionally. Employee may be required to lift over 20 lbs. Employee is occasionally required to bend, squat, crawl, kneel, climb, lift waist to overhead. Employee is occasionally required to reach forward and reach overhead. For repetitive action, employee is required to perform simple grasping and pushing/pulling. Employee is occasionally exposed to electrical hazards and CRT terminal work. (Occasionally: 1-33%, Frequently: 34-66%, Continuously: 67-100%) The most significant of duties are included but this does not exclude occasional work assignments not mentioned or developmental duties.
Master's degree in Social Work with five or more years of social work experience within a hospital or skilled nursing facility.
