Position Summary
We are seeking a dedicated and compassionate Occupational Therapist (OT) to join our team. The OT will provide skilled therapy services to patients in their homes, focusing on improving functional independence, safety, and quality of life.
Key Responsibilities
- Evaluate patients’ functional status and develop individualized care plans
- Provide skilled occupational therapy interventions in the home setting
- Educate patients and caregivers on adaptive techniques and safety strategies
- Collaborate with physicians, nurses, and interdisciplinary team members
- Complete timely and accurate documentation in compliance with Medicare and company standards
- Participate in case conferences and care coordination
Qualifications
- Current California Occupational Therapist license (required)
- Minimum 1 year of experience (home health experience preferred)
- Strong clinical assessment and documentation skills
- Ability to work independently and manage time effectively
- Valid driver’s license and reliable transportation
