Job Type: 12-hour shifts: 6:00 pm to 6:00 am, Full-Time
Schedule #1: ROTATING! Wednesday, Thursday, Friday, // Thursday, Friday, Saturday
Schedule #2: Saturday, Sunday, Monday
***$31.00 to $42.00 per hour depending on experience and license type
Requirements:
- Licensed LPN/LVN/RN
- A Fingerprint Clearance Card is Required
- CPR Certificate
- Must currently be in Arizona/greater Phoenix area
Job responsibilities:
- Conduct comprehensive head-to-toe assessments, record vital signs, monitor changes in physical and mental conditions, and document all findings in medical records.
- Administer medications as prescribed by physicians, and monitor residents for any adverse reactions or side effects.
- Collaborate with an interdisciplinary team (physicians, physical therapists, dietitians, social workers, etc.) to develop, implement, and evaluate individualized care plans for each resident.
- Act as a liaison between residents, families, and the healthcare team, ensuring all parties are informed of health status changes and the resident's wishes are respected.
- Maintain accurate and detailed patient charts and records to ensure continuity of care and compliance with all regulations.
- Respond promptly to medical emergencies, such as falls or sudden declines in condition, and provide immediate interventions. Welcome residents and orient them to the environment.
