Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or wheelchairs.
- Monitor patients’ vital signs, observe health status, and report any changes to healthcare professionals promptly.
- Prepare nutritious meals tailored to individual dietary needs, ensuring proper food safety and hygiene standards.
- Administer medications accurately following prescribed care plans while maintaining strict adherence to HIPAA regulations for patient privacy.
- Support clients with memory care needs related to Alzheimer’s disease or dementia by implementing personalized routines and safety protocols.
- Maintain detailed documentation of care provided, patient progress, and any incidents in compliance with healthcare standards.
- Perform light housekeeping tasks such as cleaning, laundry, and organizing to promote a safe and hygienic living environment.
Skills
- Proven experience in home & community care or assisted living environments with a strong understanding of patient observation and monitoring techniques.
- Knowledge of first aid, CPR certification, and emergency response procedures to ensure client safety at all times.
- Familiarity with working with individuals with developmental disabilities, autism spectrum disorder, or dementia-related conditions such as Alzheimer’s and memory care needs.
- Ability to lift heavy objects safely and operate mobility aids like Hoyer lifts or other assistive devices.
- Excellent communication skills for collaborating with healthcare teams, family members, and clients to develop effective care plans.
- Strong organizational skills for managing multiple clients’ schedules, medication administration, and documentation requirements.
- Compassionate attitude combined with a proactive approach to supporting clients’ physical, emotional, and social well-being.
