About the Role
This full‑time position provides services throughout Green Bay and surrounding areas. You’ll primarily work in a home health setting, with occasional visits in skilled nursing facilities. It’s also a great opportunity for new graduates interested in developing their skills in home health and geriatrics.
What You’ll Do
- Work independently with strong self‑direction and minimal supervision
- Review and respond to patient referrals
- Collaborate with the clinical team to educate patients and caregivers
- Develop and implement individualized treatment plans with functional, achievable goals
- Provide instruction and supervision to COTAs
- Assist with identifying and recommending appropriate DME (wheelchairs, adaptive equipment, etc.)
- Perform additional responsibilities as needed to support patient care
