Overview
We are seeking a dedicated and energetic Resident Care Coordinator to join our dynamic healthcare team. Responsibilities
- Coordinate comprehensive resident care plans by collaborating with interdisciplinary teams, including nurses, therapists, and physicians.
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to inform personalized care strategies.
- Oversee medication administration and injection procedures following HIPAA regulations and nursing policies to ensure safety and compliance.
- Manage documentation of medical records, clinical notes, and regulatory reports using EMR (Electronic Medical Record) systems such as Cerner or eClinicalWorks.
- Supervise nursing staff and care team members to ensure adherence to state healthcare regulations and long-term care standards.
- Facilitate discharge planning, family communication, and coordination with external providers for seamless transitions of care.
- Monitor infection control protocols, catheter care, tube feeding procedures, and sterile processing to maintain a safe environment for residents.
- Ensure compliance with HIPAA privacy laws while managing sensitive patient information and medical documentation.
- Lead quality improvement initiatives related to patient assessment accuracy, medication management, and nursing policy compliance.
- Support residents with specialized needs such as dementia or memory care through tailored activities and behavioral health interventions.
Skills
- Extensive knowledge of medical terminology, physiology, anatomy, ICD coding (International Classification of Diseases), and diagnostic evaluation techniques.
- Proven experience working with individuals with disabilities or developmental disabilities across various settings including assisted living or long-term care facilities.
- Strong leadership skills in supervising nursing teams and managing multidisciplinary staff within a healthcare environment.
- Proficiency with EMR/EHR systems such as Cerner or eClinicalWorks for accurate documentation and clinical workflow management.
- Ability to perform complex patient assessments involving vital signs, physical examinations, venipuncture (blood draws), catheterization, IV insertion/infusion techniques, and sterile Familiarity
- Knowledge of state healthcare regulations related to long-term care regulations and utilization review processes such as DRG (Diagnosis-Related Group) management and utilization management strategies.
- Excellent managerial communication skills for team supervision, staff training, family engagement, and interdisciplinary collaboration.
- Competence in infection control practices including specimen collection/processing, laboratory experience, infection prevention protocols, and aseptic techniques.
- Ability to handle complex cases involving acute pain management or chronic conditions like Alzheimer’s or dementia care while maintaining high standards of patient safety.
