Description: This position will support the Health Equity Coordinator with their community-based work. The Health Equity Coordinator is responsible for the managing the care of the uninsured population referred to by CVH staff.
The Peer Support Specialist will complete a Social Determination of Health (SDOH) assessment to screening for various social needs and benefit eligibility that may be interfering with their Primary Care treatment. The Peer Support will also be a critical component of this team by providing direct support to the consumers as they connect to the referred community resources. Consumers may struggle with a variety of Brain Health/Mental Health/Substance Use disorders. This position is eligible for remote work.
Complete an assessment of a member's healthcare needs, functional needs, accessibility needs, strengths and supports, goals, and other characteristics that will guide the plan for care.
Provide direct support to consumers, connecting them with various community resources
NC Peer Support certification or Community Health Worker Certification
Preferred 2 years of experience working directly with individuals with behavioral health conditions (if serving members with behavioral health needs) or with an I/DD or a TBI (If serving members with I/DD or TBI needs)
Must have valid Driver’s License and auto insurance
Must have a good driving record
Strong communication skills (written and verbal)
