The Community Liaison is responsible for developing and maintaining strong relationships with healthcare providers, facilities, community organizations, and potential referral sources to promote hospice services. This role serves as the primary representative of the hospice in the community, ensuring accurate education, awareness, and timely referrals while supporting patients and families in accessing quality end-of-life care.
Build and maintain positive relationships with physicians, hospitals, nursing homes, assisted living facilities, and community organizations to increase hospice awareness and referrals.
Conduct regular outreach visits, presentations, and educational programs for healthcare professionals and the community.
Provide timely follow-up and support to referral sources, ensuring excellent communication and customer service.
Collaborate with the clinical team to coordinate the admission process for referred patients.
Track and report referral activity, community engagement, and outreach effectiveness.
Represent the hospice at health fairs, community events, and professional networking activities.
Maintain up-to-date knowledge of hospice regulations, services, and eligibility requirements to provide accurate information.
Assist families, patients, and caregivers in understanding hospice services and benefits.
Meet or exceed established goals for referrals, admissions, and community engagement.
Bachelor’s degree in marketing, healthcare, business, or related field preferred; equivalent experience considered.
Minimum 2 years of experience in healthcare marketing, sales, or community relations, preferably in hospice, home health, or senior care.
Strong knowledge of hospice philosophy, eligibility criteria, and healthcare industry regulations preferred.
Excellent communication, interpersonal, and presentation skills.
Ability to build and sustain professional relationships across diverse settings.
Self-motivated with strong organizational and time management skills.
Proficiency in Microsoft Office and CRM or referral tracking systems.
Valid driver’s license, reliable transportation, and ability to travel within assigned territory.
Ability to travel frequently to healthcare facilities, community events, and referral sources.
May require evening or weekend availability for events or urgent referrals.
Ability to sit, stand, and drive for extended periods.
