Keystone Post Acute is a 65-bed skilled nursing facility providing short-term rehabilitation and long-term care services in Fresno, California. We are seeking a dedicated and experienced MDS / Infection Prevention Nurse to join our clinical leadership team. This position serves as a dual-role MDS / Infection Prevention Nurse, with the Infection Prevention responsibilities shared in collaboration with the Director of Staff Development (DSD).
This role works closely with the DSD, Director of Nursing (DON), and interdisciplinary team to ensure regulatory compliance, accurate reimbursement, quality resident care, and effective infection prevention practices throughout the facility.
The ideal candidate will have strong knowledge of MDS processes, PDPM, Medicare documentation, infection prevention practices, and long-term care regulations. We are looking for someone who is organized, proactive, detail-oriented, and able to work collaboratively with nursing, therapy, and administration teams to support positive clinical and operational outcomes.
Duties & Responsibilities
- Complete accurate and timely MDS assessments in accordance with federal and state regulations.
- Coordinate the RAI process including assessments, care plans, CAAs, and ARD scheduling.
- Monitor PDPM and case mix opportunities while ensuring supporting documentation is present.
- Participate in Triple Check, Medicare meetings, Clinical IDT, QAPI, and utilization review processes.
- Collaborate with nursing, therapy, dietary, and social services to ensure individualized resident care planning.
- Review medical records for compliance, accuracy, and reimbursement optimization.
- Share Infection Prevention responsibilities alongside the Director of Staff Development (DSD).
- Track and monitor infection prevention data including healthcare-associated infections, isolation practices, antibiotic stewardship, and surveillance logs.
- Conduct infection prevention rounds and audits throughout the facility.
- Provide education and guidance to staff regarding infection control practices, PPE usage, hand hygiene, and outbreak prevention.
- Ensure compliance with CDC, CMS, OSHA, and California Department of Public Health (CDPH) guidelines.
- Assist with preparation for state surveys, audits, and regulatory reviews.
- Maintain accurate documentation and reporting related to infection prevention and quality measures.
- Work collaboratively with the DSD and nursing leadership team to support staff education and quality improvement initiatives.
Qualifications
- Current California LVN license in good standing.
- Minimum 1–2 years of MDS experience preferred.
- Infection Prevention experience in a skilled nursing facility preferred.
- Strong understanding of PDPM, Medicare, managed care, and long-term care regulations.
- Knowledge of infection prevention standards, surveillance, and outbreak management.
- Experience with PointClickCare (PCC) preferred.
- Strong organizational, communication, and time management skills.
- Ability to work independently while maintaining strong interdisciplinary collaboration.
- Skilled in documentation review, clinical assessment, and regulatory compliance.
