Duties
- Conduct thorough patient assessments to evaluate social, emotional, and environmental needs, ensuring adherence to social work practice standards and HIPAA regulations.
- Develop personalized discharge plans that incorporate community resource coordination, medical documentation, and compliance with Medicare and ICD-9/ICD-10 coding requirements.
- Provide clinical counseling and psychotherapy to individuals and groups, utilizing techniques such as motivational interviewing, cognitive behavioral therapy, and behavioral health interventions.
- Collaborate with interdisciplinary teams—including healthcare providers, therapists, and case managers—to coordinate patient care plans aligned with clinical ethical guidelines and hospice & palliative medicine standards.
- Assist patients and families through crisis intervention, end-of-life care discussions, addiction counseling, and dementia or Alzheimer's care planning.
- Maintain accurate medical records and documentation of all client interactions using electronic health record systems while ensuring confidentiality under HIPAA guidelines.
- Facilitate intake assessments for new patients, including diagnostic evaluations for mental health or behavioral health concerns such as PTSD or autism spectrum disorders.
Experience
- Proven experience working with geriatric patients in acute care or assisted living environments, demonstrating familiarity with senior care protocols.
- Hands-on experience in hospital or outpatient settings involving medical social work, inpatient experience, or post-acute care environments.
- Strong background in clinical research related to behavioral health or community-based interventions is preferred.
- Demonstrated ability to manage complex case loads involving diverse cultural backgrounds and disabilities while adhering to social work practice standards.
- Knowledge of medical terminology, CPT coding (Current Procedural Terminology), ICD coding systems (International Classification of Diseases), and Medicare regulations is highly desirable.
- Experience working with individuals with disabilities or neurodevelopmental conditions such as autism spectrum disorder is advantageous.
- Certification in basic life support (BLS) or CPR required; additional certifications in hospice & palliative medicine or addiction counseling are a plus.
