Heartwood Avenue Healthcare is 60-bed skilled nursing facility located in Vallejo, CA. We are professional, customer-centric, collaborative and our goal is to provide excellent quality care for our residents through customer service and safety.
We are looking for talented Director of Nursing that will contribute to our facility culture of customer service, safety, and regulatory compliance. They will have the ability to build and maintain a strong team that works well together to achieve great results. Must have a valid RN license and one year of supervisory experience as a RN. We have a great team, who completed a 5-star recertification survey in May of 2026. We are looking for a qualified candidate with leadership skills to add to our team.
Job Duties and Responsibilities:
Clinical Leadership & Oversight
- Direct and supervise all nursing services to ensure delivery of quality resident care in accordance with physician orders, care plans, and facility policy
- Oversee 24-hour nursing coverage, staffing schedules, and appropriate skill-mix per CA Title 22 and mandated nurse-to-resident ratios
- Conduct clinical rounds and monitor resident outcomes, including skin integrity, falls, medication management, and infection control
Regulatory Compliance
- Ensure compliance with Title 22, and CMS Requirements of Participation (42 CFR Part 483)
- Maintain infection prevention and control program in line with CDPH and CDC guidance
Staff Management
- Recruit, hire, train, and evaluate licensed and certified nursing staff (RNs, LVNs, CNAs)
- Provide ongoing competency validation, in-services, and orientation programs
- Address performance issues, staff scheduling, and disciplinary actions in coordination with HR
Care Planning & Documentation
- Oversee interdisciplinary care plan development and MDS/RAI process accuracy and timeliness
- Ensure accurate, timely clinical documentation to support quality measures and reimbursement (PDPM)
- Review incident reports, grievances, and QAPI data for trends requiring intervention
Quality Assurance & Performance Improvement (QAPI)
- Chair or actively participate in QAPI committee meetings
- Track and trend clinical quality indicators (falls, pressure injuries, UTIs, rehospitalization rates)
- Develop and monitor performance improvement plans
Administrative Duties
- Manage nursing department budget, staffing costs, and supply utilization
- Collaborate with Administrator on facility operations, census, and strategic planning
- Maintain current knowledge of nursing practice standards, Board of Registered Nursing requirements, and evolving state/federal regulations
Qualifications
- Current, active California RN license in good standing
- Minimum 1–2 years supervisory nursing experience in a skilled nursing/long-term care setting
- Working knowledge of Title 22, MDS/PDPM, and CMS survey process
Job Type: Full-time
Pay: $145,000.00 - $175,000.00 per year
