About the Position
Northeastern Home Group is seeking an experienced, compassionate, and reliable Direct Support Professional to provide Individualized Home Supports (IHS) with Training and Homemaker services to individuals receiving Minnesota HCBS waiver services.
This position is ideal for someone who understands that IHS with Training is a teaching service—not simply completing tasks for the client. The successful candidate will help individuals build independence by teaching daily living skills while also providing quality homemaker services that maintain a safe, healthy home environment.
Schedule
- 32 Hours per Week
- 16 Hours – Individualized Home Supports (IHS) with Training
- 16 Hours – Homemaker Services
Individualized Home Supports (IHS) with Training Responsibilities
The employee must understand that IHS with Training focuses on skill development, independence, and measurable progress toward client goals, not doing everything for the client. Services should be individualized based on each person's Support Plan, CSSP Addendum, and person-centered goals.
Responsibilities include:
- Teach independent living skills
- Provide coaching instead of doing tasks for the individual
- Help clients learn household management skills
- Teach meal planning and cooking
- Teach budgeting and money management
- Teach grocery shopping skills
- Teach medication reminder routines (within scope)
- Teach organization and time management
- Teach community safety
- Teach transportation skills
- Teach communication skills
- Teach appointment scheduling
- Teach problem-solving skills
- Teach healthy lifestyle habits
- Teach personal hygiene routines
- Build confidence and self-sufficiency
- Support community integration
- Encourage independence rather than dependence
Examples include:
- Teaching a client how to prepare a meal
- Teaching laundry from start to finish
- Teaching how to clean a bathroom properly
- Teaching budgeting techniques
- Teaching how to use community resources
- Teaching clients to independently complete household tasks
- Tracking measurable progress toward individualized goals
Documentation Expectations for IHS with Training
The employee must be able to write detailed, person-centered documentation that includes:
- Goal being addressed
- Skill taught
- Teaching method used
- Client participation
- Level of prompting provided
- Client response
- Progress toward independence
- Outcome achieved during the shift
- Plan for the next visit
Documentation should clearly demonstrate teaching, coaching, and measurable progress, not simply task completion.
Homemaker Responsibilities
Homemaker services focus on maintaining a clean, healthy, and safe living environment based on the individual's assessed needs.
Responsibilities include:
- Meal preparation
- Kitchen cleaning
- Bathroom cleaning
- Laundry
- Vacuuming
- Sweeping
- Mopping
- Dusting
- Changing bed linens
- Trash removal
- Household organization
- Grocery assistance
- Basic household upkeep
- Maintaining sanitary conditions
- Helping maintain a safe living environment
Staff should encourage client participation whenever appropriate while ensuring services are delivered according to the client's service authorization. Homemaker services are governed by Minnesota homemaking service standards.
Documentation Expectations for Homemaker
The employee must accurately document:
- Tasks completed
- Client participation
- Safety concerns observed
- Home condition
- Services delivered
- Client response
- Any changes that need to be reported
Documentation must accurately reflect the homemaker services provided during the shift.
Qualifications
Preferred:
- Experience providing Minnesota 245D HCBS services
- Experience with Individualized Home Supports (IHS) with Training
- Experience providing Homemaker services
- Strong understanding of person-centered practices
- Excellent written documentation skills
- Professional communication skills
- Reliable transportation
- Valid driver's license and auto insurance (if transporting clients)
