About the Role
EMI Advisors is seeking a FHIR Developer for immediate hire to support federal and state health interoperability initiatives. In this role, you will design, build, and test FHIR-based APIs and data exchange solutions that conform to HL7 implementation guides such as eLTSS and US Core. You will work directly with clients, partners, and our lead architects to solve real-world integration challenges — from API security and patient matching to conformance testing and deployment automation. This is a growth-oriented position, ideal for a developer who wants to deepen their health IT standards expertise and evolve toward solutions architecture.
Key Responsibilities
- Develop, test, and maintain FHIR R4 resources, profiles, mappings, and APIs, in conformance with HL7 implementation guides such as eLTSS and US Core.
- Support implementation and troubleshooting of API security components, including OAuth 2.0 flows, mTLS certificates, and token-based authorization, working with client IT teams and external partners.
- Execute integration, conformance, and quality assurance testing against FHIR implementation guides, document results, and track resolution of defects and data quality issues.
- Support patient identification and matching workflows, including investigating and resolving matching discrepancies with client teams.
- Build and maintain CI/CD pipelines, deployment scripts, and monitoring for client environments, applying secure development and DevSecOps practices across health and non-health engagements.
- Produce and maintain technical documentation, including interface specifications, onboarding guides, test plans, runbooks, and release notes.
- Participate in client technical working sessions, providing clear status updates and escalating architectural questions and risks to the lead architect.
- Assist with data analysis and validation tasks, including reviewing FHIR payloads for completeness, conformance, and consistency with source system data.
- Grow familiarity with adjacent capabilities such as CQL, terminology services, and standard code systems through project work and structured learning.
Required Qualifications
- Bachelor's degree in Computer Science, Software Engineering, Health Informatics, Information Systems, or a related technical field
- 3+ years of experience in software development, systems integration, or health IT, with at least 1 year working directly with FHIR or comparable healthcare data standards (HL7 v2, CDA).
- Proficiency in at least one modern development language (e.g., C#/.NET, Java, JavaScript/Node.js, Python) and experience consuming and building RESTful APIs.
- Working knowledge of API authentication and authorization concepts (OAuth 2.0, TLS/mTLS, tokens); hands-on certificate management experience a plus.
- Experience with CI/CD tooling, version control (Git), and at least one cloud platform (Azure, AWS, or GCP); exposure to infrastructure as code and security scanning tools a plus.
- Familiarity with healthcare data privacy basics (HIPAA) and comfortable working in regulated or government environments.
- Strong troubleshooting and documentation skills, with the ability to communicate technical issues clearly to both technical and non-technical audiences.
- Eagerness to learn from senior architects and grow into broader solutions architecture responsibilities.
- Exposure to Medicaid, HCBS, or public sector health programs a plus but not required.
Preferred Qualifications
- Hands-on experience with FHIR servers and tooling (e.g., HAPI FHIR, Azure Health Data Services, AWS HealthLake, Firely, or Smile CDR)
- Experience authoring or profiling FHIR Implementation Guides using FHIR Shorthand (FSH), SUSHI, or Simplifier
- Familiarity with SMART on FHIR, Bulk Data (Flat FHIR), and CDS Hooks specifications
- Experience with healthcare terminology standards and services (SNOMED CT, LOINC, RxNorm, ICD-10)
- Exposure to CQL (Clinical Quality Language) or clinical quality measure development
- Experience with patient matching/master data management tools or algorithms
- Knowledge of TEFCA, USCDI, and CMS/ONC interoperability regulations
- Experience with HL7 v2-to-FHIR or CDA-to-FHIR mapping and data transformation
- Active participation in HL7 workgroups, connectathons, or accelerator programs (e.g., Gravity, Da Vinci)
- Prior consulting experience or client-facing delivery in government health programs (CMS, Medicaid, ACL)
