Job Description
We are seeking a licensed and compassionate Per Diem / PRN Home Health Physical Therapist to provide in-home physical therapy services to patients in Bucks, Montgomery, Northampton, and Lehigh counties. The ideal candidate will be experienced in home health, patient-centered care, and skilled in developing individualized treatment plans to improve mobility, strength, balance, and overall functional independence.
This position offers flexible scheduling and gas stipend
Responsibilities:
- Perform in-home physical therapy evaluations and treatments
- Develop and implement individualized treatment plans based on patient needs
- Assess patient progress and adjust treatment plans accordingly
- Educate patients and caregivers on exercises, safety, and fall prevention
- Complete accurate and timely documentation in the EMR
- Communicate effectively with physicians, nurses, and interdisciplinary team members
- Comply with Medicare regulations and agency policies
Requirements
- Current Physical Therapist license in the State of Pennsylvania
- Graduate of an accredited Physical Therapy program
- Minimum 1 year of experience (home health experience preferred)
- Knowledge of Medicare home health guidelines and documentation standards
- Valid driver’s license and reliable transportation
- Ability to work independently and manage time effectively
Schedule & Coverage Area
- Flexible PRN schedule
- Coverage needed in Bucks, Montgomery, and Philadelphia counties
