About the Role
Touchstone Home Health is seeking a Speech Language Pathologist (SLP) to join our San Antonio team—where clinical excellence meets purpose-driven care. In this role, you’ll deliver skilled, compassionate therapy that helps patients regain communication, independence, and quality of life—all in the comfort of their homes.

Key Responsibilities

Provide speech therapy services in accordance with physician orders and individualized care plans

Conduct comprehensive evaluations of communication and swallowing needs

Develop and adjust treatment plans to support patient progress and safety

Educate patients, families, and caregivers on home-based strategies and exercises

Complete timely and accurate documentation, including OASIS assessments and care updates

Collaborate with physicians and interdisciplinary teams to coordinate high-quality care
Requirements:
What We’re Looking For

Master’s or Doctoral degree in Speech Language Pathology

Current SLP license in the state of Texas (or eligibility)

Strong communication skills and the ability to connect with diverse patients and families

Adaptability in dynamic, home-based care environments

Organized, self-directed approach to managing schedule and documentation

Willingness to travel between patient homes throughout the San Antonio area
