Job Type: Contract (1099), part-time – approximately 12–13 hours per week across 48 service weeks (~600 hours annually)
Pay: $45.00 – $50.00 per hour
Schedule: Weekdays (M-F), daytime hours; flexible within Adult Care Center operating schedule.
This position is contingent upon the completion and cleared background check and TB test.
About the Program:
SASCC is a 501(c)(3) nonprofit dedicated to helping communities thrive through the demographic shift toward an older population. Our Saratoga Adult Care Center (ACC) provides structured daytime care for older adults in Saratoga and the surrounding West Valley communities (Campbell, Cupertino, Los Gatos, Monte Sereno, Saratoga, and Santa Clara).
We are seeking a contracted LCSW to deliver our Behavioral Health and Caregiver Support Program, funded by a community benefit grant. The program strengthens the behavioral health and resilience of older adults and family caregivers through caregiver support groups, behavioral health screening, individual consultation, collaborative care planning, and coordinated referrals, all within a trusted community setting.
Responsibilities
- Facilitate a weekly 1 hr Caregiver Support Group for family and community caregivers, reducing isolation and connecting participants to resources.
- Conduct quarterly structured caregiver consultations using validated assessment tools, including the UCLA Loneliness Scale (ULS-8) and the Caregiver Self-Efficacy Scale (CSES-8)
- Conduct quarterly behavioral health and quality-of-life screenings for ACC clients, with individual behavioral health consultations as needed.
- Provide referral coordination and system navigation, connecting caregivers and clients to mental health, respite, and community resources.
- Participate in monthly collaborative care-plan review meetings with the ACC Manager to support individualized client care.
- Maintain timely clinical documentation and progress notes; documentation and care coordination time is included in paid hours.
- Track service delivery and screening results to support grant outcome reporting, including quality-of-life improvement for ACC clients and caregiver burden reduction among support group participants.
Qualifications
- Active California LCSW license in good standing with the CA Board of Behavioral Sciences (required).
- Experience serving older adults, family caregivers, or adults with chronic health conditions.
- Experience facilitating support groups or group-based interventions.
- Working knowledge of validated behavioral health screening instruments (e.g., ULS-8, CSES-8).
- Strong documentation, care coordination, and communication skills.
- Bilingual candidates and experience in geriatric or community-based settings a plus.
Position Details
- Independent contractor engagement funded by a one-year community benefit grant; continuation contingent on grant renewal.
- Approximately 600 paid hours annually across 48 service weeks (~12.5 hours per week).
- Serves approximately 42 individuals annually, including family caregivers and ACC clients.
