About This Role
We are looking for a detail-oriented, experienced MDS Coordinator to lead our RAI process and drive accurate PDPM reimbursement. You will work closely with nursing, therapy, dietary, and social services to ensure assessments are complete, care plans are individualized, and our facility performs at its best. RN license strongly preferred — LPN with significant MDS experience will be considered.

What You'll Do
Complete all MDS assessments accurately and within CMS ARD windows
Submit assessments to iQIES and resolve any rejected or late assessments
Manage PDPM case mix including clinical categories, NTA scoring, and therapy minutes
Lead or co-lead interdisciplinary care conferences
Develop and update individualized, person-centered care plans
Monitor Quality Measures and contribute to QAPI initiatives
Conduct internal MDS audits to ensure coding accuracy and compliance
Educate nursing staff on documentation that supports MDS coding
Collaborate with the Business Office on Triple Check and billing accuracy
Assist with survey preparation and respond to MDS-related findings
Requirements
Current RN license strongly preferred — LPN with MDS experience considered
Minimum 2 years of MDS coordination experience in a skilled nursing facility
Strong working knowledge of PDPM, the RAI Manual, and iQIES
Experience with PointClickCare, MatrixCare, or similar EHR platform
RAC-CT certification preferred or willingness to obtain
Excellent organizational and analytical skills
Ability to meet CMS submission deadlines consistently
