Family Home Care Group is actively hiring compassionate and motivated Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to join our growing home health team.

Responsibilities
- Evaluate and treat patients in their homes according to physician orders and plan of care
- Improve patient mobility, strength, balance, endurance, and safety
- Educate patients and caregivers on exercises, fall prevention, and functional mobility
- Collaborate with nurses, physicians, and interdisciplinary care team members
- Complete documentation accurately and timely
- Participate in care coordination and patient progress updates
QualificationsPT Requirements
- Active Florida Physical Therapist license
- Graduate of an accredited Physical Therapy program
PTA Requirements
- Active Florida Physical Therapist Assistant license
- Graduate of an accredited PTA program
Additional Requirements
- Home health experience preferred, but not required
- Reliable transportation and valid driver’s license
- Current CPR certification
- Strong communication and time management skills
- Ability to work independently in the field
Preferred Experience
- Home health or geriatric experience
- Knowledge of Medicare documentation requirements
- OASIS familiarity is a strong plus for PTs
- Experience with orthopedic, neurological, cardiac, and post-surgical patients
