We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our hospice team. The Hospice LPN plays a vital role in providing high-quality, patient-centered care to individuals with life-limiting illnesses while supporting their families with dignity, empathy, and respect. Working under the direction of the Registered Nurse Case Manager, the LPN provides skilled nursing care, monitors patient conditions, and helps ensure comfort throughout the end-of-life journey.
Provide skilled nursing care in accordance with the patient's individualized plan of care.
Perform nursing assessments and report changes in patient condition to the RN Case Manager and physician.
Administer medications and treatments as ordered by the physician.
Monitor pain and symptom management, documenting patient response to interventions.
Educate patients, families, and caregivers regarding medications, disease progression, and comfort measures.
Assist with wound care, catheter care, ostomy care, and other skilled nursing procedures.
Accurately document all patient visits, assessments, interventions, and communications in the electronic medical record.
Participate in interdisciplinary team meetings and collaborate with physicians, social workers, chaplains, hospice aides, and other team members.
Respond appropriately to patient and family concerns while providing emotional support.
Maintain compliance with hospice regulations, agency policies, HIPAA, and infection prevention standards.
Participate in on-call rotation, weekends, or holidays as assigned.
Perform other duties as assigned.
Qualifications
Current, unrestricted Licensed Practical Nurse (LPN) license in the state of employment.
Graduate of an accredited Practical Nursing program.
Current CPR/BLS certification.
Valid driver's license, reliable transportation, and proof of automobile insurance.
Ability to travel throughout the assigned service area.
Excellent communication, organization, and documentation skills.
Ability to work independently while collaborating effectively with an interdisciplinary team.
One (1) year of nursing experience.
Previous hospice, home health, oncology, geriatrics, long-term care, or palliative care experience.
Experience using electronic medical records (EMR).
Ability to lift, transfer, or assist patients up to 50 pounds with or without reasonable accommodation.
Frequent standing, walking, bending, kneeling, and reaching.
Ability to travel between patient homes and healthcare facilities throughout the workday.
