Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patient conditions, noting changes in health status and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Administer medications and treatments following medical instructions while maintaining strict adherence to HIPAA privacy regulations
- Perform vital sign checks such as blood pressure, pulse, temperature, and respiration rates to track health trends
- Support clients with specialized needs such as Alzheimer's care, dementia care, memory care, and working with individuals with developmental disabilities or disabilities
- Utilize assistive devices like Hoyer lifts safely to transfer patients and prevent injury
- Manage catheter care, wound care, and hospice support as required by the care plan
- Prepare nutritious meals and assist with meal planning while maintaining food safety standards
- Conduct light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for clients
- Document all patient interactions accurately in medical records or documentation systems while respecting confidentiality standards
Experience
- Prior experience in assisted living facilities, nursing homes, or home health environments is highly preferred
- Demonstrated caregiving experience working with seniors or individuals with disabilities or chronic conditions such as Alzheimer's or dementia
- Familiarity with medical terminology and medical documentation practices
- Experience using EMR/EHR systems for documentation and patient record management
- Certifications such as CPR (Cardiopulmonary Resuscitation), first aid training, and medication administration certification are required or highly desirable
- Long-term care experience including live-in employment is advantageous
- Knowledge of hospice care principles and working with patients at end-of-life stages is a plus
- Ability to work independently while following detailed care plans in a home setting
