Position Summary
The Registered Nurse (RN) provides skilled nursing care to patients in their homes by assessing patient needs, developing individualized care plans, coordinating services, and delivering compassionate, evidence-based care. This role requires strong clinical judgment, excellent communication, and the ability to work independently while collaborating with physicians and the interdisciplinary team.
Responsibilities
- Complete comprehensive Start of Care (SOC), ROC, Recertification, Follow-up, and Discharge assessments.
- Develop, implement, and revise individualized Plans of Care based on patient needs.
- Perform skilled nursing assessments, treatments, medication management, and patient education.
- Regularly evaluate patient progress and update care plans as needed.
- Coordinate patient care with physicians, therapists, home health aides, and other healthcare professionals.
- Communicate changes in patient condition and obtain physician orders when necessary.
- Complete accurate and timely clinical documentation in WellSky.
- Provide health education and counseling to patients and caregivers.
- Supervise Home Health Aides and complete required supervisory visits.
- Participate in case conferences, quality improvement initiatives, and on-call rotation as assigned.
- Ensure patients receive appropriate equipment and services to support their care.
Qualifications
Required
- Current California Registered Nurse (RN) license
- Graduate of an accredited School of Nursing
- Current CPR/BLS Certification
- Valid California Driver's License
- Reliable transportation with current auto insurance
- Ability to travel to patient homes within assigned territory
- Strong assessment, clinical, communication, and documentation skills
- Ability to work independently with minimal supervision
Preferred
- Minimum 2 years of nursing experience
- Home Health, Public Health, Hospice, or Community Health experience
- Case Management experience
- Experience using WellSky or other Home Health EMR systems
- Bilingual is a plus

Physical Requirements
- Ability to stand, walk, bend, kneel, crouch, and lift or transfer patients as needed.
- Ability to lift and carry clinical supplies and equipment.
- Visual and hearing ability sufficient to safely perform nursing responsibilities.

Pay: $60.00 - $150.00 per hour
