Full-Time Home Care Opportunity
Redeemer Health Home Care & Hospice is seeking compassionate and skilled Physical Therapists to join our interdisciplinary team. This role involves providing high-quality, patient-centered care in home settings across Cumberland County.
Qualifications:
· Graduate of an APTA approved and accredited college of Physical Therapy
· Current Physical Therapist license in the State of New Jersey
· CPR certification required / Driver’s License required
· Strong oral, written, and interpersonal communication skills
· Demonstrated competency in physical therapy principles, methods, materials, and equipment
· Ability to assess patient needs and develop/implement comprehensive care plans
· Ability to work collaboratively within an integrated healthcare team
· Must be able to travel within the designated service area to patient homes
Key Responsibilities:
· Evaluate patients’ functional abilities and limitations
· Develop individualized treatment plans to promote independence and quality of life
· Implement therapeutic interventions and monitor patient progress
· Collaborate with physicians, nurses, and other healthcare professionals
· Educate patients and caregivers on therapy goals and home programs
