Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ vital signs and observe for any changes in health status, reporting findings accurately and promptly
- Administer medications following care plans and ensure compliance with HIPAA regulations to maintain confidentiality
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Use specialized equipment such as Hoyer lifts or other assistive devices to safely transfer clients with mobility challenges
- Maintain a clean and safe environment by performing light housekeeping tasks like cleaning, laundry, and organizing living spaces
- Support clients with cognitive impairments such as dementia or Alzheimer’s disease through memory care techniques and engagement activities
- Document care activities meticulously, including patient observations, medication administration, and adherence to care plans
- Collaborate with healthcare professionals, social workers, and family members to ensure comprehensive patient support
- Provide compassionate companionship, encouraging social interaction and mental stimulation
Qualifications
- Proven experience in home health care, assisted living, nursing homes, or group home settings; experience with long-term care preferred
- Knowledge of patient observation techniques, vital signs monitoring, medication administration, and HIPAA compliance
- Certification in CPR and First Aid required; additional certifications such as hospice or dementia care are a plus
- Ability to lift heavy objects safely using proper techniques or assistive devices like Hoyer lifts
- Familiarity working with individuals with developmental disabilities, autism spectrum disorder, or memory-related conditions such as Alzheimer’s disease
- Strong communication skills to effectively coordinate with healthcare teams and family members
- Compassionate attitude with a commitment to providing respectful and dignified care
- Flexibility to work various shifts including evenings, weekends, or holidays as needed
