**Job Summary:** We are seeking a dedicated Care Manager to join our team. The Care Manager will be responsible for coordinating patient care and providing support to individuals in need of medical assistance. **Duties:** - Collaborate with healthcare providers to ensure comprehensive patient care - Manage patient cases and develop individualized care plans - Monitor patient progress and adjust care plans as needed - Provide guidance and support to patients and their families - Conduct assessments to identify patient needs and preferences - Coordinate referrals to specialists and other healthcare professionals - Maintain accurate and up-to-date medical records **Qualifications:** - Experience in behavioral health, infectious disease care, patient care, managed care, PICU, hospital medicine, HIPAA compliance, medical records management, acute care, and primary care - Bachelor's degree in Nursing or related field preferred - Strong communication and interpersonal skills - Ability to work effectively in a fast-paced environment - Knowledge of healthcare regulations and best practices This position offers the opportunity to make a meaningful impact on the lives of patients while working in a dynamic healthcare setting.
