Position Summary:
The Team Lead is the designated point person on shift to support the Program Director in clinical and administrative oversight. Aid in ensuring proper standards and regulations are adhered to as set forth by the funding source. Help guarantee the individuals living in the house are provided optimum support and necessary care. Under the direction of the Program Director assists and directs all house staff to provide this support.
Essential Job Function:

Assists with the the day-to-day operation of the program relative to: medical appointments, community activities, scheduling, menu planning, arranging transportation, etc.

Assists the Program Director in supervision and direction to program staff regarding treatment/support plans and interventions, including rehabilitation strategies and techniques.

Under the direction of the Program Director assists with scheduling nursing and direct care staff to provide appropriate coverage and support 24-hour a day, 7-days a week.

Assists with providing outreach and create an environment in which family, friends and established relationships of the person served may be fostered.

Helps monitor the quality and timely completion of all clinical, recordkeeping and service/support action plans and necessary documentation for all persons served.

Assist Program Director to ensure compliance with all internal quality and external licensing, certification, and accreditation standards and regulations, as well as program specific policies and procedures.

Under the direction of the Program Director manages and reports adverse events (including but not limited to medication occurrences, physical restraints, or other critical incidents), including thorough documentation and follow through for all events. Help develop and implement plans of correction as appropriate or instructed.

Provide on-call backup and on-site support when necessary.

In partnership with MAP supervisor or consultant, ensure the storage, administration, documentation and disposal of medications for persons served is compliant with MAP requirement and standards, funding source regulations and agency policy.

Requirements (Knowledge and Critical Skills):

Excellent verbal and written communication skills.

Proven ability to manage a diverse staff in a 24-hour residential setting.

Exceptional organizational and time management skills.

Success developing positive therapeutic and working relationships with diverse stakeholders.

Ability to assess problem situations to identify causes, gather information and act appropriately.
- Valid driver’s license with clean driving record.

Knowledge of program management and of individuals with acquired brain injuries.
Qualifications:
Education: High School degree required. College degree strongly preferred.
Experience: 2 years of experience working in human services or allied health. 1 year of supervisory experience.
Certifications: MAP (Medication Certification Process) required (in house training provided).
