Job Title: Director of Social Services

Summary:
The Director of Social Services leads the social services department to ensure resident-centered psychosocial care, regulatory compliance, and high-quality discharge planning in a nursing home setting. Responsible for care planning, MDS coordination, utilization of PointClickCare, and oversight of social work staff.
Key Responsibilities:
- Lead and manage the social services team; hire, train, schedule, and evaluate staff.
- Conduct psychosocial assessments and document interventions in PointClickCare.
- Develop, implement, and monitor individualized care plans in collaboration with the interdisciplinary team.
- Oversee MDS-related psychosocial coding and ensure accuracy of resident assessments.
- Complete and coordinate discharge planning and transitions of care, including community referrals and placement support.
- Conduct PSARR (Psychosocial Assessment & Reassessment Review) processes and ensure timely reassessments.
- Manage UDS (Uniform Data System) reporting and related data collection as required.
- Maintain compliance with federal, state, and local regulations and facility policies.
- Serve as resident/family advocate; address grievances and facilitate family meetings.
- Provide crisis intervention and link residents/families to community resources and benefits (e.g., Medicaid, VA, SSI).
- Monitor and report quality indicators related to social services and participate in QAPI activities.
- Maintain accurate, timely documentation and confidentiality of resident records.
Required Qualifications:
- Bachelor’s degree in Social Work (BSW) or related field; Master’s in Social Work (MSW) preferred.
- Current licensure/certification as required by state for social workers.
- Minimum 1 years’ experience in nursing home/skilled nursing facility social services; supervisory experience preferred.
- Proficient with PointClickCare (clinical documentation and care planning).
- Strong knowledge of MDS processes, PSARR, UDS, and discharge planning protocols.
- Excellent interpersonal, communication, and organizational skills.
- Ability to manage multiple priorities and work collaboratively with interdisciplinary teams.
Preferred:
- Experience with regulatory surveys and corrective action planning.
- Familiarity with community resources, benefit programs (Medicaid, Medicare, VA), and placement processes.
- Demonstrated success with quality improvement initiatives.

Pay: $27.00 - $32.00 per hour
