Job Title: Nurse Assessor (RN) – Home Health
Location: Bronx - Morrisania / Fordham-Belmont / Baychester / Univ. Heights - E. Tremont / Parkchester, NY (Hybrid) - Candidate can choose one location
Duration: 12 Months Contract + Extension Opportunity (Up to 5 Years)
Pay Range: $54 - $55/hr + $90/ weekly for mileage
Schedule: Day Shift | Hybrid ( 3 Days Onsite, 2 Days Remote)
Hours: Up to 40 Hours/Week
ONE MONTH REMOTE TRAINING OF UAS
Required Qualifications:
- Active RN License in New York State.
- Minimum of 2 years of Home Health experience within the last 15 years.
- Recent experience conducting comprehensive patient assessments.
- Strong clinical assessment, documentation, and care coordination skills.
