Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems such as Point click Care.
- Administer medications and injections safely, including IV infusions and catheter care, adhering to HIPAA and clinical confidentiality policies.
- Manage acute pain and chronic conditions through evidence-based nursing interventions, including wound care, tube feeding, and ventilator management.
- Coordinate patient care plans, discharge planning, and case management while collaborating with physicians, therapists, and other healthcare professionals.
- Conduct diagnostic evaluations such as specimen collection/processing, medical imaging coordination, and diagnostic testing like spinal taps or cardiac catheterizations.
- Provide specialized care for geriatric patients with dementia or Alzheimer's disease in memory or long-term care settings.
- Assist in emergency situations with airway management, trauma response (Level I/II trauma center experience).
- Document all patient interactions accurately in EMR/EHR systems while complying with healthcare regulations including HIPAA.
- Engage in health coaching and education for patients with chronic illnesses or post-acute conditions to promote optimal recovery and health maintenance.Experience
- Proven experience working in diverse healthcare environments such as hospitals , skilled nursing facilities, hospice care, or outpatient clinics.
- Demonstrated expertise in medical-surgical nursing, geriatrics, internal medicine, pediatrics, or behavioral health nursing.
- Knowledge of ICD-9/ICD-10 coding standards for accurate medical documentation; experience with CPT coding is a plus.
- Skilled in IV insertion/insertion techniques.
- Strong understanding of long-term care regulations and infection control practices within skilled nursing facilities or assisted living communities.
