Responsibilities
- Deliver comprehensive personal care assistance, including bathing, dressing, grooming, and mobility support
- Assist with Activities of Daily Living (ADLs) such as toileting, feeding, and medication reminders
- Monitor patient health by observing vital signs and reporting changes promptly using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Support patients with mobility using patient transfer methods and equipment like Hoyer lifts or gait belts while ensuring safety protocols are followed
- Administer medications accurately in accordance with medical documentation and home healthcare regulations
- Provide specialized care for patients with dementia or Alzheimer’s disease, including memory care techniques and behavioral management
- Maintain patient confidentiality by adhering to HIPAA guidelines and safeguarding medical records and sensitive information
- Assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to promote a safe and comfortable environment
- Support end-of-life care with compassion, dignity, and respect for patients’ wishes
- Communicate effectively with clients and their families to ensure understanding of care plans and address concerns
Qualifications
- Proven experience working as a Home Health Aide or in assisted living/nursing home settings preferred
- Strong knowledge of medical terminology, medication administration, catheter care, and vital signs monitoring
- Experience working with individuals with developmental disabilities or dementia is highly desirable
- Familiarity with EMR/EHR systems for documentation and client communication
- Certification in First Aid and Basic Life Support (BLS), including CPR training required; additional certifications in hospice or geriatrics are a plus
- Ability to lift heavy objects safely and perform physically demanding tasks such as patient transfers using Hoyer lifts or other equipment
- Knowledge of home health care regulations and compliance standards such as HIPAA is essential
- Excellent communication skills to foster trustful relationships with clients and their families
- Valid driver’s license and reliable transportation preferred for client visits
