Position: Full-Time Hospice Admissions Registered Nurse (RN)
What You’ll Do:
- Conduct hospice admission visits in homes, hospitals, and care facilities
- Perform comprehensive nursing assessments and hospice eligibility evaluations
- Educate patients and families about hospice philosophy, services, and expectations
- Coordinate with referral sources, physicians, hospitals, and our interdisciplinary team
- Complete timely and accurate clinical documentation
- Serve as a calm, confident clinical leader during emotionally significant moments
What We’re Looking For:
- Active Colorado RN license
- Hospice and/or home health experience strongly preferred
- Strong assessment, communication, and clinical judgment skills
- Compassionate bedside manner and emotional intelligence
- Ability to work independently while remaining closely connected to the team
- Reliable transportation for local travel
- Current CPR certification

Qualifications:
- Registered Nurse licensed in the State of Colorado
- Demonstrates strong observation, problem-solving, verbal, and written communication skills
- Ability to independently organize and prioritize workload
- One year of recent medical/surgical experience and/or hospice experience strongly preferred
- Valid driver’s license, insured reliable transportation, and good driving record required
