Schedule: Day Shift | PRN
Salary: $40.00 - $42.00 per hour
What You Will Do:

Perform comprehensive diagnostic assessments of patient’s needs and psychosocial status.

Develop a comprehensive, individualized treatment plan and goals for the patient and provide psychotherapy, group and family therapy if applicable.

Collaborate with other members of the multidisciplinary team, as appropriate, to develop treatment goals for the patient.

May participate in multidisciplinary patient care conferences discussing information obtained from patient assessments, updates, treatment plans.
What You Will Need:
Licensure / Certification / Registration:

One or more of the following required:
-
Clinical Counselor credentialed from the Illinois Department of Financial and Professional
Regulation obtained prior to hire date or job transfer date. -
Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

Licensed Clinical Social Worker (LCSW) credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or transfer date.

Licensed Clinical Professional Counselor (LCPC) credentialed from the Illinois Department of
Financial and Professional Regulation obtained prior to hire date or job transfer date. -
Social Worker specializing in Clinical Practice credentialed from the Illinois Department of
Financial and Professional Regulation obtained prior to hire date or job transfer date.

BLS Provider preferred. American Heart Association or American Red Cross accepted.
Education:
-
High School diploma equivalency with 2 years of cumulative experience OR Associate's
degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
Additional Preferences:
- Master's Degree required

LPC or LCPC licensure required
