Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic home healthcare team. Duties
- Assist the Registered Nurse in carrying out the plan of Care.
- Provide skilled nursing care, under the direct supervision of the Registered Nurse.
- Provide wound care, catheterization, tube feeding, and sterile technique procedures to promote healing and prevent infections.
- Support patients with complex needs such as dementia care, Alzheimer's care, memory care, and behavioral health management.
- Foster cooperative effort, among personnel, by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.
- Document all patient interactions thoroughly using Wellsky, formerly Kinnser, while maintaining strict compliance with HIPAA confidentiality policies.
- Educate patients and their families on health management strategies including pain management techniques, medication administration, and health coaching.
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification required.
- Proven experience working with geriatric patients, individuals with disabilities, or those requiring specialized dementia or memory care.
- Strong knowledge of physiology, anatomy, medical terminology, and clinical assessment techniques.
- Familiarity with infection control practices and basic life support (BLS).
- Excellent communication skills for caregiver collaboration and client education; experience working with individuals with developmental disabilities is advantageous.Join us in delivering exceptional patient-centered care that transforms lives! Pay: $39.00 - $40.00 per hour
