Responsibilities:
- Supervise the planning, coordination and implementation of a structured Social Adult Day Care program for seniors with Alzheimer's type dementia and medical frailty.
- Conduct participant assessments and write care plans
- Assist with intake screenings and visit days.
- Maintain communication with all families, and managed care organizations (MMLTC).
- Provide direct supervision to staff to ensure that the goals of maintaining function, overall safety, and social/emotional support are maximized for all participants.
- Coordinate staff scheduling in order to ensure adequate staff.
- Lead or assist with Caregiver Support Group and all special events related to Day Haven.
- Supervise food service and ensure that the program is in compliance with all Senior Meal requirements.
Qualifications:
High School Diploma required; bachelor's degree preferred.
1 year experience in elder care or related field required.
1 year supervisory experience preferred.
