Responsibilities
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to ensure patient stability
- Support medication administration following proper protocols and documentation standards while maintaining HIPAA compliance
- Observe and report changes in patient condition promptly to healthcare teams to facilitate timely interventions
- Provide companionship and emotional support to reduce feelings of isolation, especially for individuals with dementia or Alzheimer's disease
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment
- Document all care activities accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to maintain comprehensive medical records
Skills
- Proven experience working with individuals with developmental disabilities, dementia, or in hospice care settings
- Strong knowledge of medical terminology and procedures related to patient care and safety standards
- Ability to perform patient observation effectively and recognize signs requiring urgent attention
- Experience operating medical equipment such as Hoyer lifts, blood pressure cuffs, thermometers, and other vital sign monitoring devices
- Familiarity with home & community care practices, including working with EMR/EHR systems for documentation
- Excellent communication skills to collaborate seamlessly with healthcare teams and family members
- Valid CPR certification along with training in catheter care, medication administration, and hospice or memory care preferred
