Join our compassionate healthcare team as a Home Health Aide (HHA), where you will play a vital role in delivering personalized, high-quality care to individuals in their homes. Duties
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support
- Monitor and record vital signs including blood pressure, pulse, respiration, and temperature using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems
- Support medication administration following prescribed protocols while maintaining HIPAA compliance for patient confidentiality
- Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care techniques and behavioral management
- Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively
- Assist with meal preparation, feeding, laundry, light cleaning, and maintaining a clean and safe environment
- Document medical records accurately and promptly while adhering to healthcare documentation standards
- Support hospice care patients by providing compassionate comfort measures and emotional support in accordance with care plans
Requirements
- Proven experience in home health care, assisted living facilities, nursing homes or long-term care environments
- Knowledge of medical terminology and procedures related to patient observation and medical documentation
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or hospice care are a plus
- Experience working with individuals with developmental disabilities or dementia is highly valued
- Ability to work independently while following detailed care plans and protocols
- Strong communication skills to collaborate effectively with patients, families, and healthcare teams
- Familiarity with EMR/EHR systems for accurate recordkeeping and reporting
- Valid driver’s license preferred for live-in employment or community-based visits
