Position: RN Care Coordinator

Duration: 6-month contract to hire
Location: Yulee, FL 32097 ( first month training at downtown location )
Hours: 8am-4:30pm
Pay Rate: $34 - $45 ( dependent upon experience)
Must-Haves:
- RN with Care Management experience (2+ years ideal)
- Strong payer / insurance knowledge
- Registered Nurse License
- Care Management experience
- Experience leading or contributing to interdisciplinary rounds
- Discharge planning experience
- Creating and updating care plans

Day-to-Day:
Registered Nurse Care Manager supports patients after they leave the hospital or a short‑term rehabilitation facility. The focus is on keeping patients safe, stable, and connected to the right medical care as they recover at home.
You work closely with physicians, primary care teams, specialists, home health, palliative care, social work, and community partners to help patients avoid complications and prevent unnecessary readmissions.
Overall purpose: Improve patient outcomes, reduce avoidable readmissions, and support long‑term wellness through ongoing, relationship‑centered clinical care.
Pay: $33.00 - $35.00 per hour
