New Concepts is expanding our youth behavioral health services and seeking exceptional Case Managers to join our dedicated team. As a Case Manager, you will play a vital role in supporting individuals and families navigating complex social systems. You will be responsible for developing personalized care plans and coordinating services to ensure the best outcomes for children and families. The Case Manager provides Therapeutic Behavioral Services (TBS) for youth aged 5–18 and their families.

Duties
- Provide client-centered care that is trauma-informed, timely, effective, and equitable.
- Collaborate with a multidisciplinary team to develop and implement individualized service plans.
- Plan, monitor, and evaluate interventions to address the specific needs of youth and their families.
- Deliver services in each individual’s natural environment (home, school, workplace, or community).
- Maintain accurate, timely documentation and uphold all agency, state, and federal standards of care.
- Build and sustain relationships with families, schools, and community partners to support youth outcomes.
Qualifications
Required:
- High school diploma or GED with at least five (5) years of related experience or a bachelor's degree with one (1) year of related experience.
- Valid driver’s license with a satisfactory driving record.
- Successful completion of a background check.
Preferred:
- Bachelor’s degree in social work, psychology, or a related behavioral health field.
- Experience working with youth in behavioral health care settings.
Job Type: Full-time
Pay: $22.00 - $27.00 per hour
