Job Summary
The Registered Nurse - Long Term Care (RN-LTC) provides comprehensive nursing care in a long-term residential setting. This role ensures resident safety, comfort, and dignity while managing their complex medical, physical, emotional, and psychosocial needs. The RN-LTC functions in accordance with established policies, regulatory standards, and nursing best practices, promoting a high level of resident satisfaction and quality of life.
Essential Functions

Performs nursing assessments and reassessments based on resident needs

Provides direct care and administers medications and treatments per protocols

Maintains timely and accurate documentation of resident care and conditions

Communicates effectively with residents, families, physicians, and interdisciplinary team members

Develops, updates, and evaluates individualized plans of care in collaboration with residents and families

Assesses and manages resident pain levels; educates residents/families on pain management strategies

Applies infection control practices and maintains a safe, clean environment

Assists residents with activities of daily living, including hygiene, mobility, nutrition, and elimination

Provides emotional support and advocates for resident rights and dignity

Participates in resident admissions, discharges, and transfers

Reports significant changes in resident conditions and follows up as appropriate

Functions as a liaison between residents, families, healthcare providers, and facility leadership

Participates in staff orientation, training, and performance improvement activities

Flexible with scheduling and willing to work as needed for departmental coverage

Qualifications
Education
-
Associate Degree in Nursing (ADN), required

Bachelor of Science in Nursing (BSN), preferred
Licenses/Certifications

Current state RN license or current Compact State RN license

Basic Life Support (BLS) Certification (AHA) required within time frame specified in facility policy
Experience

Previous long-term care or geriatric nursing experience preferred
