Mountain Comprehensive Care Center is hiring a Targeted Case Manager to serve children and adults in. This position supports clients diagnosed with Serious Mental Illness (SMI), children/youth with Serious Emotional Disturbance (SED), and individuals with Substance Use Disorder (SUD) who are active in treatment services.
Targeted Case Managers help clients and families access resources, coordinate care, improve stability, and work toward individualized goals. This role requires strong communication, independent judgment, timely documentation, and the ability to work as part of a treatment team.
Position Overview
The Targeted Case Manager provides additional support to clients by identifying needs, coordinating services, connecting clients to community resources, and monitoring progress toward care plan goals. This position works closely with clients, families, clinical staff, schools, medical providers, social service agencies, recovery supports, and other community partners to support continuity of care and improved quality of life.
Key Responsibilities
The Targeted Case Manager will:
- Document client history, including skill deficits, basic support needs, level of functioning, risk status, substance use/recovery needs when applicable, and service needs.
- Assess and prioritize client strengths, barriers, risks, and needs.
- Gather collateral information from medical providers, educators, social workers, family members, natural supports, recovery supports, and other relevant sources.
- Develop person-centered care plans that reflect the client’s goals, needs, preferences, recovery needs, and measurable objectives.
- Complete prior authorization documentation and submit required information to support service approval and continuation.
- Coordinate services and refer clients to appropriate community resources and support organizations.
- Document direct and indirect contacts with clients and collateral supports.
- Complete progress notes that clearly describe services provided and client progress toward care plan goals.
- Communicate relevant updates, concerns, and case information with clinical staff to support quality care.
- Monitor care plans, progress notes, activities, and client contacts in collaboration with the clinical services supervisor.
- Attend care plan meetings and participate in treatment team collaboration.
- Maintain an active caseload of approximately 25 clients.
- Complete all required documentation in a timely and accurate manner.
- Complete additional duties as assigned.
Qualifications
· Minimum: Bachelor’s degree in human service field, (i.e. Psychology, Social Work, Counseling) with a minimum of one year experience. Minimum of one year Case Management experience required or a Master’s in human service may substitute.
