Community health center based in Jersey City is in need of Licensed Practical Nurses (LPN). Being proficient in Spanish and English is preferred, but not a deal breaker. The LPN assists in the implementation of the nursing process on assigned patients. The practice of nursing as an LPN includes performing tasks and responsibilities within the framework of vaccine administration for children and infants, and providing provisions of supportive and restorative care. Nurse will be assigned to assist with patients and school physicals. Please note that salary listed is opening rate, but experience and credentials could be negotiated. Experience in Pediatrics is welcomed, but not a deal breaker. Must have superior clinical skills and knowledge of vitals and documentation and EMR experience. Being proficient in manual vitals and documenting them is key to the success in this position.
Additional duties include:
Implements care in a knowledgeable, skillful, consistent and continuous manner, taking into consideration the age, growth and development of the individual.
- Assists physician with vital signs, weight, lab retrieval, documentation, maintain logs for daily reports i.e. mammography x-ray, etc.
- Prepares patients for vaccinations .
- Reviews physician’s notes/lab requests and reports.
- Provides health education based on patient’s needs. Documents patient response to treatment/teaching in the medical record.
- Observes, reports and documents significant physical, psychological or safety status changes in patient’s condition to the RN/PSC/Provider promptly.
- Communicates patient care needs and refers to ancillary staff and follow-up accordingly.
- Administers medication therapy and vaccines as ordered by the provider.
Implementation of patient plan of care
- Follow Unit policy on plan of patient care and age-specific assessment.
- Carries out specific function at the direction of the Charge Nurse or Nurse Dir.
- Obtains necessary equipment and assembles it in examination room as needed.
- Assures that department and rooms are appropriately stocked for patient care.
- Understands and demonstrates Standard Precaution measures in all patient care areas.
