NURSE PRACTITIONER (Part-Time)
Home-Based Primary Care
Service Area: Miami Dade and Broward County, Florida
Summary of Position:
We are seeking a compassionate, experienced, and motivated Nurse Practitioner to join our growing home-based primary care practice. Our mission is to provide high-quality, patient-centered medical care to seniors with multiple chronic conditions in the comfort of their homes, helping them age safely in place while reducing avoidable hospitalizations and emergency department visits.
Duties will include but are not limited to, disease management, performing physical exams, recording patient medical histories, and creating patient care plans.
This position requires daily travel to patients' homes, assisted living facilities, and other residential care settings within the assigned service area.
Key Responsibilities:
- Provide extensive primary care to patients to help the patient age in place as appropriate and reduce both unplanned hospitalization and unnecessary emergency department visits.
- Assist the practice in meeting ACO quality measures
- Time management
- Triage incoming messages, completed within the same day as the message was received
- Complete all progress notes within 24 hours of the visit
- Perform physical exams and patient observations
- Record patient medical histories and symptoms
- Create patient care plans and contribute to existing plans
- Order, administer and analyze diagnostic tests
- Monitor and operate medical equipment
- Diagnose health issues
- Detect changes in a patient's health and change treatment plans as necessary
- Involve community resources as needed
- Evaluate responses to treatments and medications
- Consult with healthcare professionals
- Train patients and families on managing illnesses or injuries and disease prevention
- Maintain quality, productive relations with practice facilities and vendors
- All other duties as assigned
Qualifications
Required
- Master's or Doctor of Nursing Practice (DNP) degree from an accredited Nurse Practitioner program.
- Current Registered Nurse (RN) license in the state of Florida (or compact state, where applicable).
- Current Florida Advanced Practice Registered Nurse (APRN) license.
- Board Certification as a Family Nurse Practitioner (FNP) through ANCC or AANP.
- Active DEA registration and prescriptive authority.
- Minimum of one (1) year of home-based primary care experience.
- Fluent in both English and Spanish.
- Valid Florida driver's license with reliable transportation.
- Current automobile insurance.
- Ability and willingness to travel daily throughout the assigned service area.
- Ability to lift and carry up to 25 pounds.
Preferred
- Experience in geriatrics, home-based primary care, hospice, or palliative medicine.
- Strong clinical assessment and chronic disease management skills.
- Excellent communication and documentation abilities.
- Experience with electronic medical records (EMR).
Schedule
- Part-Time
- 8-hour daytime shifts
