South Mountain Post Acute is seeking Arizona-licensed LPNs to join our rehabilitation and sub-acute units on the night shift. We are a Medicare 5-Star Rated Skilled Nursing Facility known for strong teamwork, a positive culture, and consistent leadership.
This role is ideal for LPNs who thrive in a fast-paced clinical environment and value collaboration, stability, and high-quality patient care. Experience with vents and trachs is a plus, but not required.

Key Responsibilities:
- Provide direct nursing care to residents under RN supervision
- Administer medications and treatments per physician orders
- Monitor resident conditions and report changes promptly
- Assist with admissions, discharges, and care coordination
- Deliver compassionate, patient-centered care in rehab and sub-acute settings
- Maintain accurate documentation in the electronic health record
Qualifications:
- Active Arizona LPN license
- Skilled nursing or sub-acute experience preferred
- Strong clinical judgment and communication skills
- Team-oriented with a commitment to quality care
