Duties
- Conduct comprehensive patient assessments, including nutritional analysis and diagnostic evaluation to determine specific dietary needs.
- Develop and implement individualized care plans that incorporate medical nutrition therapy (MNT), and nutrition support strategies.
- Provide nutrition counseling using motivational interviewing techniques to motivate behavioral change and enhance patient engagement.
- Collaborate closely with healthcare professionals such as physicians, nurses, and therapists to coordinate patient care and optimize treatment outcomes.
- Manage electronic health records (EHR/EHR systems) efficiently, documenting assessments, care plans, progress notes, and follow-up actions accurately.
- Educate patients on nutrition principles related to chronic disease management, geriatrics, weight loss, and acute care needs.
- Supervise dietetic interns or junior staff members while ensuring adherence to hospital or long-term care facility standards.
Skills
- Extensive knowledge of physiology related to digestion, metabolism, and nutritional requirements across diverse populations.
- Proven experience in clinical nutrition, including medical nutrition therapy (MNT), dietary supervision, and working with individuals with specific dietary needs.
- Strong background in health coaching techniques such as motivational interviewing to foster positive lifestyle changes.
- Familiarity with EMR/EHR systems for documentation and patient record management.
- Skilled in developing care plans for complex cases involving tube feeding, nutrition support, or specialized diets.
- Demonstrated ability to communicate effectively with clients of varied backgrounds and collaborate seamlessly with multidisciplinary healthcare teams.
- Experience working within hospital settings or long-term care facilities with a focus on geriatrics and chronic disease management.
