Job Summary:
Assumes responsibility for collecting and verifying the accuracy of birth certificate worksheet data and entering and transmitting this data to local State and County registrar within the prescribed time-frame. Reviews patient records and codes birth certificates accordingly. Collects and reviews all paternity forms. Submits paternity forms to State and County registrars. Works with sensitive and confidential information.
Job Requirements:
Education and Work Experience:
- High School Education/GED or equivalent: Preferred
- Associate’s/Technical Degree or equivalent combination of education/related experience: Preferred
- One year's relevant experience: Preferred
Department Specific License/Certifications:
- Basic Life Support (BLS) certification from approved vendor per AH policy: Required
Essential Functions:
- Collects and ensures all documentation on birth certificate worksheets are complete and accurate.
- Reviews medical records and codes each birth certificate accordingly.
- Enters all initial data and amendments for birth certificates, transmits to Health Department within the allocated time frame and submits all worksheets to HIM.
- Collects, reviews and sends Paternity documentation forms from patients to State and County monthly.
- Performs other job-related duties as assigned.
