Overview:
We Are seeking a dedicated and experienced Registered Dietitian to join our healthcare team. The ideal candidate will provide expert nutritional care, develop personalized care plans, and support patient health across various settings including hospitals, long-term care facilities, and outpatient clinics. Duties
- Conduct thorough patient assessments to evaluate nutritional status and identify needs.
- Develop and implement individualized care plans focusing on weight management, disease-specific nutrition, and recovery strategies.
- Provide specialized nutrition therapy including tube feeding management, dietary modifications, and diagnostic evaluation of nutritional deficiencies.
- Offer health coaching to promote lifestyle changes related to diet, weight loss, and overall health improvement.
- Monitor patient progress and adjust care plans accordingly, ensuring optimal outcomes.
- Collaborate with medical teams to integrate nutrition strategies into overall treatment plans.
- Supervise dietary department staff and oversee meal planning in accordance with clinical guidelines.
- Utilize Electronic Medical Records (EMR) systems for documentation and communication of patient care activities.
- Educate patients on nutrition-related topics, including geriatrics, acute care, long-term care, and specialized diets for various medical conditions.
Experience
- Proven experience as a Registered Dietitian in hospital or healthcare settings is required; long-term care experience is highly preferred.
- Demonstrated expertise in physiology knowledge, dietetics principles, and clinical nutrition practices such as tube feeding and weight loss programs.
- Supervising experience in dietary departments or healthcare teams is advantageous.
- Familiarity with EMR systems for accurate documentation and patient tracking.
- Experience working with diverse patient populations including geriatrics and those requiring acute or chronic care management.
- Skills in patient assessment, creating care plans, dietary modifications, and diagnostic evaluation are essential.
- Prior experience with hospital-based nutrition services or in settings involving dietary department operations enhances suitability for this role.
