We are seeking a dedicated and energetic Physical Therapist to join our dynamic team. Duties
- Conduct comprehensive evaluations of patients’ physical conditions
- Develop individualized care plans targeting specific needs and goals
- Deliver hands-on care with therapeutic exercises
- Educate patients and their families on injury and fall prevention, recovery strategies, and home & community care routines
- Coordinate continuity care planning to ensure seamless transition from hospital or SNF to home
- Maintain accurate and timely documentation within electronic medical record (EMR) systems for all patient interactions and progress notes
- Collaborate with multidisciplinary teams to optimize patient care and achieve treatment goals
Experience
- MUST have a PT license in Virginia, or multistate
- Proven experience working in environments with a strong understanding of acute care protocols
- Demonstrated success in outpatient clinics or home health settings with diverse patient populations
- Knowledge of medical terminology relevant to therapy practices and patient assessments
- Hands-on experience with geriatric populations
- Familiarity with EMR systems for efficient documentation and patient record management
- Prior involvement in discharge planning, care plan development, and community-based care initiatives

Pay: From $120,000.00 per year
