Job Summary
We are seeking a dynamic and compassionate Case Manager to join our dedicated healthcare team. Duties
- Conduct thorough patient assessments, including medical history review and social determinants of health to develop tailored care plans.
- Coordinate with healthcare providers, social services, and community resources to ensure comprehensive support for patients.
- Manage utilization review and utilization management processes to optimize hospital stays and outpatient services.
- Facilitate discharge planning by arranging follow-up care, home health services, or hospice care as appropriate.
- Document all case management activities accurately within EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems while adhering to HIPAA regulations.
- Communicate effectively with patients and their families, providing education on medical terminology, treatment options, and available social services.
- Monitor patient progress and maintain accurate documentation.
- Educate patients about treatment plans and available services.
- Perform otehr administrative duties as assigned
- Utilize critical care experience, hospital experience, and knowledge of CPT coding, ICD-9/10 coding to support clinical documentation and billing processes.Experience
- Proven experience in medical field case management or utilization review
- Strong background in medical, nursing or social work with familiarity in behavioral health, adult day care, hospice care, and discharge planning.
- Demonstrated expertise in working with managed care organizations and understanding health insurance policies.
- Proficiency in EMR/EHR systems and Microsoft Office tools for documentation and communication purposes.
- Knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, ICD-9/10 coding for accurate clinical documentation.
- Experience working with individuals from diverse cultural backgrounds with sensitivity and professionalism.
- Excellent client communication skills with the ability to handle intake processes effectively in fast-paced environments such as emergency medicine or hospital medicine units.
- Bilingual abilities are a plus to serve a broader patient population effectively.
- Preferred 4 years experience
