The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a skilled nursing facility under the supervision of a Registered Nurse (RN) or Director of Nursing. The LPN is responsible for administering medications, monitoring resident health, assisting with care plans, and ensuring high-quality care in accordance with facility policies and state regulations.
Provide direct nursing care to residents in accordance with care plans and physician orders.
Administer medications and treatments as prescribed.
Monitor residents’ vital signs, conditions, and responses to treatment.
Report changes in resident condition to the Registered Nurse or physician.
Assist with the development and implementation of individualized care plans.
Supervise and support Certified Nursing Assistants (CNAs).
Maintain accurate and timely medical documentation.
Ensure compliance with infection control and safety standards.
Respond to resident needs and emergencies.
Participate in shift reports and interdisciplinary team meetings.
Provide compassionate care while maintaining resident dignity and privacy.
Current Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) license in the state of employment.
Graduate of an accredited practical nursing program.
Experience in long-term care or skilled nursing preferred.
Current CPR certification required.
