As a Physician, you will be responsible for coordination of proactive care for patients with chronic conditions as well as identifying wellness and/or preventative service for clinical patients. In addition to clinical time, this position will also serve as faculty to our Physician Residency Program.
JOB RESPONSIBILITIES PHYSICIAN:

Utilizes appropriate and effective patient care plans which includes adequate diagnostic procedures, therapies, use of consultation and referrals, and outcomes.

Manages patients to assure continuity of care which includes adequate scheduling and tracking of return visits, completion of treatment plans, and follow-up of chronic problems, abnormal tests, and “no show” appointments.

Provides whole-person care which includes appropriate

referrals to the social worker, nutritionist, and mental health professionals.

integration of biologic, social, psychological and environmental issues.

Maintains adequate, concise, and legible documentation of patient care.

Provides and documents appropriate periodic health examinations. (i.e. preschool vision exams, serum cholesterol, pap smears, etc.)

Educates patients about their diagnosis, treatment, prognosis, medications, preventive care, etc.

Utilizes appropriate infection control measures, including universal precautions.

Oversees residents in the management of hospital patients.

Writes daily progress notes in hospital charts.
-
Takes phone calls about hospital patients.
-
Assists in the development and maintenance of HealthLinc practice guidelines (protocols) as assigned.

Participates in Total Quality Management (TQM) activities.
- Attends meetings and hospital section meeting as scheduled.

Attends required in-service meetings.

Teaches students and residents as assigned.
JOB RESPONSIBILITIES FACULTY:

Assists in arranging rotations necessary to meet the program goal and objectives.

Counsels’ residents in academic matters.

Participates in the on-site program reviews.

Participates in the recruitment and selection of new candidates.
